Jorge Caballero, D.O., graduated from San Diego State University and received his Doctor of Osteopathic Medicine at Western University of Health Sciences in Pomona, California. John Muenz, M.D. is a board certified physician with experience in physical medicine and rehabilitation, pain management and addiction management. Dr. Muenz has served patients for more than 20 years in private practice as well as supervising primary care providers for Baptist Health System, St. Vincent’s HealthCare System and other independent family practitioners.

Jordan Phelps, D.O., is a dedicated and accomplished physician specializing in Primary Care Sports Medicine and Family Medicine. He obtained his Bachelor of Science in Kinesiology from Louisiana State University in Baton Rouge, Louisiana, graduating with Cum Laude honors. He received his Doctor of Osteopathic Medicine from Nova Southeastern University/KPCOM in Davie, Florida.
Dr. Phelps completed his residency in Family Medicine at UCF/HCA GME Consortium in Gainesville, Florida. Here, he honed his skills in providing comprehensive care for patients of all ages. He went on to complete his fellowship in Primary Care Sports Medicine at HCA/Memorial Health University Medical Center in Savannah, Georgia.
Dr. Phelps has provided expert medical coverage as a team physician, and has been involved in the healthcare of the Georgia Southern football and basketball teams, Chatham County Public Schools football teams, iKon FC, Tormenta FC, Savannah College of Art and Design’s athletic teams, Walt Disney World Marathon, and the Savannah Bananas baseball team.

Katherine Hamilton, MSN, APRN, FNP-C, ENP-C is a board certified advanced nurse practitioner, family nurse practitioner and emergency nurse practitioner. She earned her Master of Science in Nursing from Jacksonville University after serving as a paramedic lieutenant for Bradford County Emergency Services, Florida. After ten years of working in emergency departments for several large Jacksonville hospitals, Hamilton transitioned to family practice and joined Family Medical Centers.
