Family Medical Centers San Pablo Welcomes New Provider

JACKSONVILLE, Fla., November 6, 2017 – Family Medical Centers welcomes a new practitioner to their San Pablo location.

Advanced Registered Nurse Practitioner Jaclyn Taylor earned her Master of Science in Nursing, Family Nurse Practitioner specialization, at the University of Alabama at Birmingham and her Bachelor of Science in Nursing from the University of Florida.

“We are adding nurse Taylor to our team to better serve the demands of our San Pablo location,” said Family Medical Centers Frederic Porcase, D.O. “Taylor will be instrumental in providing preventative and acute care for patients of all ages.”

Prior to joining Family Medical Centers, Taylor worked for Brooks Rehabilitation Hospital, developing her interest in improving the care of pediatric, adult, and geriatric patients with chronic disabling conditions. Taylor has also served as a home health care nurse, conducting geriatric assessments of physical, social and environmental health. She performed extensive wound care, IV infusion therapy, ostomy and post-surgical care.

“I have an individualized, holistically-minded approach to patients, and seek to exceed my patients’ expectations of health and well-being,” said Taylor.

Taylor’s professional affiliations include Sigma Theta Tau Honor Society of Nursing, Nu Chapter, the American Association of Nurse Practitioners and the Association of Rehabilitation Nurses. Taylor connects with her community as a volunteer with the Duval County Council on Aging’s Meals on Wheels Program and with Lutheran Social Services’ Refugee Services Program.
